Sz.P. means Szanowna Pani, Szanowny Pan or Szanowni Państwo in Polish. In this case (like in other formal letters directed to a man, woman or a couple) it's proper to write this on the envelope but...

All would be understandable in Poland, but these bottom two are the correct ones.
The "ul." is just the abbreviation for "ulica" - "street" in English - often used when writing an address in Poland.
