Usually employer sponsor the health insurance as part of company policy.
you should have travel and medical insurance at the time of visa application.

1. Yes, travel insurance same as medical insurance but there are limited insurance company letters consider for National VISA.
you have to check the site for those company names.
2. For National Visa, you have to get the 3 months of insurance otherwise possibility of rejection of VISA.

you can submit the application in polish embassy in mumbai or delhi (its depends on your citi/state).

1. once you get national D visa then you can apply the dependent visa for your spouse and child, for supporting documents you need to go through poland embassy in india website, Also no need to specify any specific visa type - just specify the dependent(in work-other option )in the form.

2. if you are plan to apply your work visa from India then in that case, you and your spouse will apply the application simultaneously.

1. You will get work permit from poland employer if you want to work as an employee.
or
2. there are some guide lines to get the work permit for those business people to setup the business in poland but for that you need to discuss(about the minimum investment,etc.) with lawyer.

To get the work permit letter from local office to employer it would take around 60 days.
30 to 60 days(check the website) to get the appointment in India for VISA application submission and after that embassy would take min 2 weeks for processing your VISA.
