In english Old Soul means someone who is wise and has possibly lived a past life. What are the Polish equivalents?

it's stara dusza just as pawian said - it happens to be the direct translation ( old soul is a pretty simple concept that's probably why)

dusza człowiek - soul man - a kind hearted, friendly person.

dusza na ramieniu - with your soul on your shoulder - scared, frightened

wytrząść duszę z kogoś - shake the soul out of sb - beat sb or kill sb

dusza towarzystwa - the life and soul of the party -

reincarnation is simply reinkarnacja in Polish
twin flame doesn't have good sounding Polish translation - a direct translation would be bliźniaczy płomień but I never encountered it
wędrówka dusz is an obsolete term (a translation of the greek word metempsychosis)

No, it is still in use, although in formal contexts, for example in titles of books which are translated from English, like the Journey of Souls.
