Just like bulletproof vests. They protect vital organs. That's the point of wearing such vests. That killed paramedic was stabbed near his heart.

All the stuff you are writing about collapsing veins and rotting teeth is because these people are using dirty needles and smoking nasty aluminum pipes - not because of the heroin.
Collapsing veins are mainly due to numerous injections. This can occur also in hospital conditions.And as for teerh and gum problems:

Portugal just decriminalized drugs completely
