Trying to find full text of a prayer that I only know fragments of

My mom used to say a prayer all the time. It was short, but I can't remember the whole thing. These are the bits I can recall

bog ze mna
bog z nami zawsze
...
do królestwa niebieskiego

Does anyone recognize this prayer? If so, can you post the full text?

Local Poles often have their own modified prayers that have never even been published in books and are only known/popular in a given region or even a village. I suspect that's the case here.
