Hello everyone! I need some assistance in obtaining my Polish passport. I reside in Columbus, OH. Which Consulate do I need to travel to to apply? I assume I need an appointment. I have my translated birth certificate and Conformation of Polish Citizenship. I do not have a PESEL number, but have been told I can apply for that at the same time as my passport. When I was applying for a passport in Canada 3 years ago I needed proof of citizenship, had to complete application form in Polish - no English version and they didn't translate it for so be prepared to read and write in Polish. You'll need passport photo to Polish standard and maybe Cash, Consulate in Canada didn't accept plastic only paper money.

I'd recommend going to the Chicago consulate - that's probably closest to Columbus - about 5-6 hour drive. And yes, you'll need an appointment especially now with covid. When I renewed mine I gave them some documents, 2 passport photos, answered a few questions on a form, paid some money (i think it was like $100-$200) and got my passport about 2 months later.
