USA, Canada / Obtaining Polish passport in the US? What do I need to do? [4]

Hello everyone! I need some assistance in obtaining my Polish passport. I reside in Columbus, OH. Which Consulate do I need to travel to to apply? I assume I need an appointment. I have my translated birth certificate and Conformation of Polish Citizenship. I do not have a PESEL number, but have been told I can apply for that at the same time as my passport.
