I am from India too and looking for the same information as you were, an year back :)
Did you get some useful information about the dependant VISA and its application process ?

Will be really thankful if you could please share your experience ?

Law / Dependant Visa for my wife (going first time to Poland) [39]

Hello Nam0011,

I am sure you would know that you both have to apply for the National VISA for Poland.
To apply the same you first need to book an appointment date at the following website : secure.e-konsulat.gov.pl/
Once you get the appointment date next step is to fill an online registration form right after taking the appointment date.

The BIGGEST challenge is to get the appointment dates. They open the slots twice a month (1st and 15th of every month).
Make sure you both sit together and keep trying to get appointment date bcz there are very limited slots available.

Keep the following information handy while filling up the online form:
Your passport details
Work location in Poland (Address)
Address of the accomodation for your initial/permanent stay in poland.
Duration of Stay


After filling in the form you need to go to Embassy of Poland in person and submit the following documents:

For your husband:
Work permit
Offer Letter
Accomodation & Flight details (dummy tickets and hotel reservations can be provided by the travel agents at some cost)
Travel Insurance for 3 months
Passport original and its photocopy
Demand draft of Rs 4400 (drawn in favour of 'Embassy of the Republic of Poland', payble at Delhi)


For you (dependent)
Your Original passport and its photocopy (plz make sure your husband's name is mentioned in your passport under Spouse details)
Your husband's passport's photocopy
Marriage certificate
Your husband's offer letter and work permit photocopy
Your husband's VISA application reference number (just in case your husband has already applied for the VISA)
Bank account statements for last 6 months
Accommodation and flight details (make sure agent issue the same flight and hotel in dummy bookings as mentioned in your husband's application)
Demand Draft of Rs 4400 (drawn in favour of 'Embassy of the Republic of Poland', payble at Delhi)
Your travel insurance (remember that the duration of the VISA is same as that of your travel insurance, if you are planning for a long stay talk to some insurance agent to get you the travel insurance for that period in advance)


I guess that's pretty all stuff that you may require while applying for both the VISAs.
