Merged: Applying for Visa with Work Permit, do I need to go in person? [USA]

Hi all -

I live in the USA and will soon receive my Polish work permit via the courier.

To apply for the VISA, do I just need to send the required documents in to the D.C Embassy or do I need to go in person? Furthermore, will I need to send health insurance forms and 'funds to stay,' if I am working in Poland?
