czyj, czyja, czyje mean whose (masculine, femine, neutral/plural form)
który, która, które mean which (quality description, which of some amount of items) (masculine, femine, neutral/plural form)
jak mean how (
adverb description)
jaki, jaka, jakie mean which kind (adjective description) (masculine, femine, neutral/plural form)
czy means if (like in sentence: I asked him if he's going to a party)
Examples:
How is she like? -
Jaka ona jest?
How is she? -
Jak z nią?
Which one is she? -
Która to? or
Która z nich to ona?
What time is it? -
Która godzina? (literally: which hour is it?)
Whose baby is it? -
Czyje to dziecko?

czy makes it more polite in this case)
Note that many of those setntences don't translate word by word into English and that's your biggest mistake that you think that if you take an english sentence and translate it word by word you will have correct sentence. This mistake is called "linguistic carbon copy" and it's pretty often for unexperience language learners whatever language their are learning.
Questions which in English start with do/does,are/is/am,has/have,will have no question word at the beginning of the sentence.
Musisz jutro wstawać wcześnie? - Do you have to get up early tommarow?
Mind that if at the end of this sentence there had been no question mark, it would has been an imperative statement:
Musisz jutro wstawać wcześnie! - You have to get up early tomarrow!
