Yes Swierczynskiego became Aleje Solidarnosci. I just checked and the building numbers are the same. The post code is: 00-145

So the address should be:

Warszawa

By sheer coincidence, I once looked at buying a flat in that building! It's that huge block facing north.

Looking at the address on Google maps, you may want to address two letters. There are two streets with that name around Warsaw, one of them in a place called Otwock and the other in Blizne.

I don't want to sound discouraging, but that address (72 m40) is the equivalent of 'No. 72, Flat 40' and both of the streets in question look like they are houses rather than blocks of flats. I'm not sure that either of them have a number 72. Are you sure it is Warsaw?

Why not try writing? Polish families don't move house as often as British people. Even if he's not at that address any more there is a very strong chance that the present occupier is a relative. That's how it works there.

The address by the way is: Ul. Swierczewskiego. not 'NL', and his first name is Grzegorz rather than Gregory.

Don't worry about the 'emerging from decades of state surveillance' thing. Poland was quite different to the Soviet Union even in those days, and now there is no special reason for discretion at all.

They didn't have it when you lived here. After EU acession it replaced the Karta Pobytu Tymczasowa for citizens of EU countries.

And he doesn't have to pay for state health insurance,well at least not directly.

He has to pay ZUS if he wants state coverage and is living in PL.

No. The permanent and temporary cards are for non-EU residents. As an EU citizen you can register with a 'Karta Obywatelstwo' but you might find that the health insurance isn't any cheaper than private insurance and the quality of service far lower. You can also stay as long as you like.

The OP should save his money. I've both used agents and done it myself. Going through an agent doesn't guarantee it's hassle free, and putting a sensibly worded ad on Gumtree is usually a better option.

That makes sense. They shouldn't block your visa card though - I have a UK one and they've never blocked in in PL. If you use a bank machine though, never, ever, agree to the fixed rate some of them suggest. It is a scam.

Depends on the job of course, however it's well above average in the provinces and not too bad in Warsaw. You'd end up taking home about 6k.
