EU pet passport / vet in Poland (Wroclaw)?

Can anyone recommend a vet (in Wroclaw) who can issue pet passports ? We are looking to ship out our cats from Poland and it seems we might need to get an EU pet passport (or alternatively a pet health certificate).

I think any registered vet can do it - the only thing you may have to do is to translate and notarize the translation.

I'm not sure about this.

Any vet in Poland can issue you an EU pet passport. It actually is the pets health records. (photo is optional). I have had one for my dog for 2 years, and he has made 3 crossings from the USA to Poland. We have also used it when travelling by car from Poland to other countries in Europe.

You should also check out chipping requirements. I had to get my cat chipped before I could import him from Belgium.
