My surname "Kandziolka"

For my whole life I've been told that my last name is Polish, but I can't find any information on it that ties it to Poland accurately. I've also been told by a Polish native that Kandziolka actually sounds more Ukrainian than Polish. Does anyone know?

Is it possible that it got changed?
There is a Polish last name "Kądziołka" (over 2300 Poles with that surname).

My maternal grandmother was Annie Kandziolka on her marriage certificate dated September 12, 1914 in Dana, Saskatchewan. Records of her birth we were told were in Warsaw(Poland) and unavailable due to WWII bombing. She married Peter Perehinski (ukrainian)of Torhowcycia, Galica. The two districts were probably neighbouring.

NOTE Torhowycia/Torhovitsia is located 5.5 miles SSW from Horodenka. It is located in the Ivano-Frankiv'sk oblast (area). Up until 1939 it was named, Stanislav. The raion (district) is Horodenkivskiy.
