spelling Kotylak right

My family name came from Poland Originaly, the bit that was Ukrainian for a while, then the family moved to the actual Ukraine. BUT the name goes back hundreds of years in Poland, so how is it SUPPOSED to be spelt. Maybe Kadylak??? And if the village was Poland then Ukraine, now Poland again, where do I look for records? Village now called Radoszyce in Sanok Provence. Found it on a map, but that's all. Any help at all would be great. Regards.

There are 271 Poles with this name. Probably more.

Both Kotylak and Kadylak exist in Poland but have different distribution patterns and are probably derived from different roots. However, it cannot be ruled out that somewhere along the line one or the other got misspelt in that pre-literacy era and confused. Would you have these names written down in any Old World documents in your possession?
