PESEL number form. Could you tell me what this means and what I need to fill in?

I have the official application form for PESEL number and need to fill in everything in Polish.
Could you tell me what this means and what I need to fill in:

V. Podstawa prawna upovazniajaca numeru PESEL - wskazanie konkretnego przepisu, z ktorego wynika obowiazek posiadania numeru PESEL: ....................................

I am guessing - but sounds like it wants your existing social security number - if you don't have one in Poland then from your home country.

It means that you have to show that you are entitled to and obliged to have PESEL (give the reference of the legal clause).

Thank you for your reply. Can you tell me what this ¨legal clause¨ is? Never applied for a pesel number before and it seems to be a complicated procedure. Especially because we do not know the language and need to fill in everything in Polish.

Does someone happen to know where exactly in Krakow we need to apply for a pesel number? I read somewhere on this forum that they were sent to a tiny office where no one spoke English.

And what do these words mean:

1. Imiona Kolejne?

2. nazwisko rodowe? I found it means maiden name so I would think that my husband does not need to fill in anything here?

3. kod teritorialny?

Once you've got a valid residency document for more than 3 months, just go to the local Urzad Miasta that deals with registration issues.

No need to fill anything in - they'll do it all for you.

I have no clue... If you are Polish citizen, this is given automatically. I think you need to ask at the place where you are applying.

3. kod teritorialny? - I think this is the post code, I never before saw this expression

It is - all you need to do is ask there. Depending on the place, it will either be granted immediately or it'll take a couple of weeks.

Wherever you got the zameldowanie from - just go there :)
