Yeah, I think I've seen this one somewhere...
Apart from the fact that it is not phonetic, this is terrible Polish/English presented and it is inaccurate and misleading to every bold guy trying to speak some Polish.

I wonder who did this...

Something like: CHEE-HO-TSKEE.
(that's actually the English pronunciation)

My Polish grandmother use to say that her mother's maiden name was pronounced ORT. WE only know the phonetic pronunciation but would the actual Polish name so that records can be researched for genealogy. Thanks

Maybe the name you're looking for is Art? To my surprise there's more than 150 people with this surname in Poland - mostly in the central area:
