Do you mean a private company (not a governmental agency)? If so, there are no rules - you talk to the company manger or owner and you establish the details between yourselves. It's like a regular business talk. On the other hand, if you mean a relocation to a job provided by the government in Poland, it's another story - they probably have a protocol and should give you more information before you are relocated.

Yes again.

Some international companies may have their own policies however on the whole very few businesses provide an expat package now unless you're in strong enough position to negotiate one.

There are no rules and regulations "politics" about relocation packages for foreigners. Some companies offer them and some don't. It's a matter of negotiations. Usually companies mention a relocation package in their job description as a benefit. Not in great detail but it will most likely say "relocation package" if there is anything on offer.

The contents of the package vary greatly. Some companies simply offer the flight to Poland. Others offer 2 - 4 weeks in a hotel on top of the flight. Others would offer the flight, hotel for a certain amount of time plus a lump sum to be paid to you. Other companies just book the flight for you and put you up in a company apartment for 1, 2 or even more years. It really depends on the company, the job you are applying for and your negotiation skills.

For example this Russian IT consulting company says:

We provide full visa and relocation assistance for you and your family. We will help you to relocate to Krakow with support for booking flights, accommodations and moving services! We offer a full relocation allowance package that includes initial accommodation expenses, real estate agency services, and compensation for tickets to Krakow.

So as people have mentioned above, if you are experienced IT specialist, then most international companies advertises some kind relocation package. You can read about it on their websites or in job offers. While if you apply to smaller company you may get nothing without asking.
