What to do with EU Visa and Poland flight ticket when surname is changed?

My Ukrainian friend has visa to Poland and flight ticket. But she got married and changed her surname and passport meanwhile. What's the procedure for such situations?

For flight best to contact the travel agent, but both, visa and flight ticket is issued to a person, not to a name (otherwise someone bearing same name could use it, which is illegal) - so I'm sure that if she can show her old documents, the EU visa will be stamped in her new passport. With flight she might only bring her old passport with, but best to check with travel agent or airlines.

Passport numbers normally stay the same.

When she was buying ticket online she had to type name only, not password number.

She will probably need to enter her passport number to print off her boarding pass.

Need your advice...

Me and my wife are from India and we both have Polish Resident cards (Karta Pobytu). Till now, surname of my wife on her Passport was her maiden name and hence we issued her Karta Pobytu with the same name (obviously). Currently she is in India for some time and we are planning to change her Surname (after marriage) and also add my name as Spouse name in her Passport. If this happens, will she be allowed to travel to Poland with this new passport (with new surname) while Karta Pobytu with old one? Is any special permission or something required? For sure, she will be travelling with Old+New Passports, Marriage certificate and how about the name on her Flight ticket? Should that be as per resident card or New passport?
