Process involved in the transfer of a work-permit?

Unless the law has changed very recently, you cannot transfer work permits.

Guys....I'm in process on changing my job in Poland and currently I'm working here based on work-permit issued from my current organization. I would like to understand if it's possible to transfer my current work-permit to my potential new employer and if yes, how?

No. Your new employer would have to apply for a work permit for you from the very beginning. The work permit belongs to your employer, not to you.
