I believe you mean the PESEL number. If you have registered your birth certificate then you should already have a PESEL number. You should contact the office where it was registered and ask them for it.

I applied for my Polish citizenship (my father is a Polish citizen) through a lawyer in Poland. I was granted the citizenship and I am awaiting for my Polish birth certificate. I wish to apply for the Polish passport but need the PESEL. I have the PESEL form all filled up (in Polish) but do not know where to send it (I live near Miami, Florida). When I called and talked to the consulate in Washington DC, the lady at times was nice and at times was short with me and just said that I need to apply for the PESEL before I can apply for the passport. I do not think I got the PESEL with the birth certificate.
