Law / Regarding Poland's Visa application of Minor child [2]

Dear all, my query is regarding Visa application of minor child.. I don't want to take my child (4 YO ) to consulate. Can I submit her application form as well.. consulate rules says yes, but need to attach authorization letter (pl see pic attached).. anyone know WT is the letter about..TIA

Hello guys, I have been trying to get appointment slot for Mumbai consulate to apply for National Visa Other category since 2 months , but unable to see any dates on Monday. I can only see the message LACK of AVAILABLE DATES TO 2018-11-02. Any suggestions on which days slots will be open. Your response is very helpful.
