Work / Difference between an employee and a contractor working in Poland [8]

I was wondering what rate of tax a person pays if working as a contractor? Also, does a contractor pay for pension and/or health insurances to the State?

What are tax and insurance rates charged via the payroll (a) to the Company and (b) to the employee, under a normal employer /employee arrangement?
