Information needed about a Polish Medal with ribbon

Hi, I just found this forum and am hoping to get some help. I am looking for information on a medal a friend gave me. I do have photos, but cannot figure out how to post them. Best I can tell, the front of the medal has this engraved on it, please excuse me, some of the letters may be incorrect, on the inner circle it has "ozolem ojcz znie szponem wrogowh", also, it has these 4 letters in a larger type "L H P S, the reverse has "czuwaj". Thanks in advance

this 'czuwaj' thing makes me think that it's some scouting organization medal
