A few more questions (about Warsaw bars, dress code, conversations)

Hi,

I have a few more questions regarding my December Warsaw trip.
A lot of people have mentioned a dress code to get into the clubs in Warsaw. What kind of clothes are acceptable? Is a nice shirt and jeans OK?

Also, do people tip waiters/waitresses, bar-tenders, etc? If so, how much is an average tip?
Is striking up a conversation with a person in a club or bar OK, especially with girls, or is this viewed as too direct or unordinary?

I appreciate any help. I don't want to make a complete fool of myself while I'm in town.

Brand clothes, that's important in Poland :). Tips aren't very popular yet. I'm not sure about picking up girls..
