This book is great. It has both examples and exercises (self-tests). The verbs are fully conjugated in all the tenses and are alphabetically arranged.

I have never heard of Czas na czasownik so I am unable to compare the two.

It is becoming more and more common for learners of language to be taught by a native speaker by using Skype. With Skype you have the ability to use video and can instantly send documents.

You could try to search polishforums.com for anything related to this or you could begin a thread looking for a Polish native interested in such a venture.

delphiandomine: I remember a visit to Poland years ago; I frequented a mBank and promised to open an account only to be able to use their free internet access. This lasted two weeks until they finally made me fill in a form, which I refused to do. So much for free internet access.
