Registering in Poland foreign birth certificate with name change?

When I was 5 my parents changed my last name from my dad's to my mum's. I've always gone by my mum's name and any document I've held has been in this name. I went to register my foreign birth certificate in the Polish office of Civil status and come across a problem because in Australia they have a newish policy where instead of giving you a separate change of name certificate and an updated birth certificate (like they used to) they list it all on the birth certificate (i.e. name given at birth and current name). The lady at the office had no idea what to do but said I may need to have my name registered as my dad's, then legally change it in Poland back to my current name. Is this the case? I've heard that people get to even have their married names as the name listed on their Polish birth certificate? I have a similar issue with my son because when he was born I gave him a really gross name (all sleep deprived from being a new mum) and when he was a few months old changed his entire name (first, middle and last names). Now not only would I rather avoid having him registered by the previous name, it would be next to impossible to get his dad to fill in any required paperwork. Has anyone dealt with something like this or knows anything more? The lady said they will be having a conference or something and she will ask there but it doesn't look too promising. If I plead with her, do you think she'd put it as my current name? Any advice please and thank you

Any pleading will not work. Probably your only way is to go by person to the Civil Office and submit a written request to the head of the registry office, and also providing your officialy translated foreign birth certificate. Then just wait for a decision. If they'd need some additional papers, try to ask for an assistance in the Polish Consulate in Australia.
