Not necessarily. The true is, you can be theoretically something between 50/50 % and 100 % Polish. Do you know why this is the right answer?

"german" and "polish" are not discrete biological categories... like most large ethnic labels there's a fair amount of admixture from other groups.

biological reductionists might come up with dumb answers like 75% Polish and 25% German, but that's not how genetics work.

more important questions:

where and how did you grow up?

what language(s) did you naturally acquire while growing up?

where feels like 'home'?

where feels like 'hmmm nice, but not home'....

Most likely Polish or strongly pro-Polish German I guess. It depends on your experiences, world view and what you are most comfortable with. Where do you feel most welcome, where do you feel at home etc.

It is up for you to decide, and the surroundings to concur with it or not

So she says. It's a bit of a weird question for a Polish person to ask but she might be very young, early teens maybe and being teased by school mates or something about not being really Polish because she has a German grandparent. Hope not. My advice, if that's the case, is tell them to bog off.

Doesn't sound very likely given how many German names show up in Poland... unless it's a very PiS heavy area (which is not Warsaw).
